Personalisation Huddle: Remote Monitoring

24 Apr 2025, 09:30 – 11:00

Remote monitoring is transforming healthcare in care homes by allowing real-time tracking of residents’ health without the need for frequent in-person visits.

Care homes and GPs can now keep an eye on critical health indicators such as vital signs (blood pressure, temperature, pulse) and pick up sudden changes that might signal deterioration or the onset of illness.

Care Homes in Lincolnshire using Whzan Remote Monitoring Tools have seen a 21% reduction in conveyancing compared to Care Homes that have not embraced this technology yet.

Our Virtual Wards patients also benefit from Remote Monitoring technology, supporting new ways of working and better health outcomes for our patients in Lincolnshire.

The potential for digital technology to enhance personalised care is incredibly exciting. It enables individuals to stay connected with their healthcare teams and leverages data to facilitate proactive care.
