Join our Personalisation Huddles to hear fellow health and care professionals and people with lived experience, from across Lincolnshire and beyond, talking about a range of topics that bring personalised and strengths-based approaches to life. ​

These sessions are available to anyone working or volunteering in health, care, and wellbeing across Lincolnshire, so why not book on, bring a brew, and enjoy the conversations? It's All About People!

Personalisation Huddle - Pathway to Person Frailty

Listen to three Primary Care Networks (PCNs) who came together at the Personalisation Huddle - Pathway to Person: Personalising frailty with local communities and neighbourhoods ​​in November 2025 ​​​​​​to showcase how local neighbourhood teams are transforming support for people living with frailty through personalised, community‑based approaches.

South Lincolnshire & Rural PCN shared how they have spent the past three months laying strong foundations for change. Their teams have focused on planning, preparation and shared learning to build a proactive neighbourhood model that connects specialist nurses, social prescribers and community partners around what matters most to the individual.

K2 PCN highlighted their work with people who have complex clinical needs, alongside the Frailty Virtual Wards. By combining comprehensive geriatric assessments with a close partnership between GPs, care coordinators and hospital frailty teams, they are keeping more people safely at home and reducing avoidable hospital admissions. Their approach emphasises listening carefully to each person’s goals and ensuring the whole system responds to them.

South Lincoln PCN, one of the county’s trailblazers, reflected on their early adoption of personalised, strengths‑based practice. Their neighbourhood teams have embraced holistic assessments, early identification of risk, and proactive support planning—helping people stay independent for longer and improving outcomes across the community.

Together, all three PCNs offered rich insights into their challenges, successes, and learning so far. Although each network approaches frailty differently, their shared commitment was clear: to move from a “pathway” mindset to a person‑centred one, grounded in partnership, prevention and understanding what truly matters to local people.
