Mapping personalisation across Lincolnshire and beyond

Our It's All About People Personalisation Champions use the Padlet mapping tool to record where they, and the people they work with and encounter in their own work, are using personalised care approaches in their practice, and making an impact in Lincolnshire.

The map also records where our Champions are actively spreading the word about personalisation, in Lincolnshire, nationally, and internationally.

We call these personalisation interventions and developments 'ripples' - and we're working with our Champions and other stakeholders across the county to turn these ripples into waves of real and lasting change.
