We would like to invite you to join our Personalisation Huddle.
The huddle is an informal catch up of like-minded colleagues in health, social care and the voluntary sector to gain an understanding of what’s happening in the Lincolnshire Personalisation Programme. We aim to update on a specific area at each huddle i.e. social prescribing, self-management, etc. We plan on bringing in colleagues from Lincolnshire and other areas across England to showcase best practise in personalised care.
The huddle is once every six weeks via MS Teams for 1.5hrs and the agenda will be sent out 2 weeks in advance. Feel free to drop in for all or the part of the huddle.
The next Personalisation Huddle is at 9.30-11am on Friday 7th July 2023. The featured topic is Social Prescribing and Community Based Support
Hear from colleagues working in social prescribing across the county, plus stories from those who have used the service.
Get an overview of who does what and where, find out about innovative local developments and the impacts of social prescribing on people and on the health and care system.
There will also be time for questions and discussion.
Email lhnt.itsallaboutpeople@nhs.net to be sent a calendar invitation

The plans don’t often capture what’s important to the person and they don’t always make sense to them. Sign up for this Personalised Care and Support Planning programme to learn about Personalisation approaches in health and care, the importance of knowing what matters to people, Lincolnshire Initial Conversation Model and much more.
