What is a Social Prescribing Link Worker?

A Social Prescribing Link Worker's job is to spend time with people and ask 'what matters to you?'

Link Workers support people to identify their priorites and interests, and then connect them with services or groups that meet their needs.

In Lincolnshire, some Social Prescribing Link Workers are employed to work with people who have specific needs or health conditions (eg people experiencing mental ill health), but most can work with anyone aged 18 or over who may need a bit of support to get started.

Link Workers are not ongoing support workers, their involvement is short term.  Their role is to link people to other sources of help in a way that works for each person.  This could be anything from providing information to accompanying someone the first time they try something new.
