Personalisation at the heart of a new era for Adult Care in Lincolnshire

Lincolnshire is entering a new chapter of Adult Social Care. Lincolnshire County Council’s draft Adult Social Care Strategy 2025–2028 sets out guiding principles, and a bold vision, for how care and support will be transformed across the county.

Due to be formally approved and published in November 2025, the strategy introduces a new way of working - one that is person-centred, strengths-based, and co-produced with people who draw on care and support.

A vision of independence

At the heart of the strategy is a simple but powerful vision:

"Empowering people to live as independently as possible in their own homes and communities"

This means focusing on people’s strengths, not just their needs. It means working alongside people, making decisions together, and actively supporting independent living.

This vision is underpinned by values that promote:

  • The building of trust and equal partnerships.
  • Making decisions together with people.
  • A culture of learning, adapting, and improving.
  • Creativity and flexibility in support.
  • A focus on what really matters to people.

These values are reflected in the “I Statements” from the Making It Real framework,  that were developed by Think Local Act Personal (TLAP), to ensure care is shaped by those who use it.

The "I Statements" are also endorsed by the Care Quality Commission (CQC) and the Local Government Association (LGA), giving them strong national recognition.

Our Shared Agreement

Our Shared Agreement – creating a better relationship between care and support and the people of Lincolnshire

Our Shared Agreement sets out a shared vision for Lincolnshire’s Integrated Care System (ICS), driven by a new kind of relationship between people and the services that support them.  

This vision, and the Five Foundations that guide and drive it, were co-produced by partners from across the county’s health, care, and wellbeing providers and people with lived experience.

Launched in July 2024 at the It's All About People Conference, Our Shared Agreement belongs firmly to Lincolnshire, and encourages all of us - providers, people, and communities - to play our part and join the movement to create meaningful change in the county.

Adult Social Care strategic priorities

The draft Adult Social Care Strategy outlines 14 strategic commitments for 2025-28. These include:

  • Co-production: Involving people with lived experience in shaping services at every stage.
  • Personalisation: Tailoring care to each person’s strengths, goals, and preferences.
  • Prevention: Expanding early support to reduce reliance on statutory services.
  • Digital innovation: Promoting technology-enabled care and AI tools to give people more choice and control.
  • Workforce development: Investing in training, leadership, and staff wellbeing.
  • Accommodation services: Expanding supported living and extra care options.
  • Safeguarding: Strengthening early intervention and person-centred protection
  • Transitions: Supporting young people with complex needs into adulthood.

A new approach: The Target Operating Model (TOM)

To meet future challenges, Lincolnshire Integrated Care System is working towards a Target Operating Model (TOM), a multi-layered approach to identifying and supporting people proactively, using personalised and strengths-based approaches across all layers. 

The TOM integrates formal and informal support across the county, focusing on:

  • Early intervention and reablement.
  • Technology-enabled care.
  • Person-centred planning.
  • Prevention before statutory assessment.
  • Collaborative working across services.

This model aims to deliver more integrated, efficient, and proactive care, responding to increasing demand and complexity.
