People’s Stories Template – measuring what matters

We know that so many services - and the personalised care they provide - make a real difference to people’s lives, supporting choice, independence, and wellbeing. Those personal outcomes don’t just matter to individuals, they help people live well for longer.

And while there’s lots of appetite for sharing these stories, it can sometimes be hard to show the bigger impact they have on health and care.

That’s why the It’s All About People Personalisation Programme Team has teamed up with partners from across health and care to create a People’s Stories Template.

It’s a simple way to capture the human impact of personalised care - the changes that numbers alone can’t always tell - while also linking those stories to the wider picture.

What will this template help you to do?

With this template, you can:

• Show the value of personalised care through Social Return on Investment (SROI).

• Identify and analyse themes and insights to share learning across the health and care system.

• Connect the impacts of personalised care with the productivity and sustainability of health and care services across Lincolnshire as a whole.

At its heart, this is about making sure people’s voices are heard, the value of their stories is captured and understood, and showing how person-centred and strengths-based approaches to health and care have a very real impact on people's lives.
