New tools to help you embed personalisation in your everyday practice
The It's All About People Personalisation Programme team is thrilled to launch two brand-new tools designed to support staff, services, and organisations across Lincolnshire in bringing personalised, strengths-based care to life in everyday practice.
Personalised Care Maturity Assessment Tool
First up is the Personalised Care Maturity Assessment Tool, a hands-on, reflective self-assessment that helps teams explore where they are now with personalised care and, crucially, where they want to go.
Rooted in Our Shared Agreement and the Five Foundations, it’s about being honest, curious, and taking smart, purposeful steps to put people at the centre of everything we do.
