Personalised Care Support Planning  - PCN Workshop

Colleagues from Primary Care Networks told us that they have to work around the systems and processes to be able to record what matters and what's important to people, as the clinical system templates are not fit for purpose and don't meet the PCSP quality assurance criteria. One of the reoccurring frustrations for people is having to repeat their story to a range of professionals: stories that can be trauma inducing and shouldn’t need to be repeated.

The Personalisation team led a workshop with partners from 9 Primary Care Networks at the New Life Conference Centre in Sleaford on 31st January 2024 about Personalised Care and Support Planning (PCSP).

We looked at;

  1. 1.       The Information Standard for PCSP

  2. 2.       Myth busting about the Shared Care Record / Care Portal and PCSP

  3. 3.       The NHSE Quality Assurance Framework for PCSP

  4. 4.       Learning and Development – changing the conversation

From 1st February, the information standard for PCSP becomes mandated across NHS organisations, to ensure that there is consistency in where, how and what is recorded.

We now have an information standard working group that has the responsibility for ensuring Lincolnshire’s NHS organisations are compliant. The group has agreed that to meet the standard, consistent adoption of the Care Portal and the Shared Care Record is required. This includes the introduction of the ‘in context’ switch in system 1 which will take colleagues straight to the Care portal without a separate log in.

The funding for this work has been approved and Primary Care are top of the list.

During this workshop we looked at what needs to be consistently recorded in a PCSP.

shared care record info graph.JPGAnthony Gorman from the digital team updated the workshop about the upgrade of the Care Portal and the Shared Care record and helped to bust some myths about recording what matters to me plans (personalised care and support plans) directly onto the care portal.

anthony.gorman@nhs.net  has offered to run some demonstrations with teams about the care portal / PCSP, however he does leave at the end of April, so you need to get in soon!

The session about the Quality Assurance framework, enabled colleagues to start thinking about their level of maturity in PCSP and what actions they needed to take when they were back in their wider teams.

Karen Atkin, interim neighbourhood lead for Grantham and Sleaford PCN’s, said; ‘the workshop had helped to identify what they needed to do in their PCN to reignite the importance of PCSP.’
