The It’s All About People team is excited to launch the new Personalisation in Action library. This growing collection of stories shows what becomes possible when care is shaped around what truly matters to each person.
These stories highlight how person-centred approaches not only improve health and wellbeing, but also strengthen individuals, families, and communities. In turn, this helps to create a health and care system that is more resilient and sustainable for the long term.
The stories we’re collecting shine a light on the many ways personalisation is taking root across health and care in Lincolnshire. And they come from part of the system - social care teams, hospitals, voluntary organisations, neighbourhood groups, and more.
Some ‘stories’ appear as reports or articles, others as podcasts or brief snapshots of everyday practice. What they all have in common is the moment someone feels genuinely heard and involved in shaping their own care and support.
As we’ve been collating and analysing the stories, several themes have emerged, including:
We’re just at the beginning – at time of writing we’ve published 24 stories, though many more are being prepared for publication. And as more of our colleagues across the system adopt personalised and strengths-based approaches, new stories will continue to surface - and we’re committed to collecting them, analysing and drawing learning from them, and sharing them.
As this library grows and evolves, we know it will offer ongoing ‘best practice’ inspiration for practitioners, leaders, and communities across Lincolnshire.
What’s clear from the stories we’ve gathered so far is that some of the most powerful examples of personalised care happen quietly, in ordinary moments. These stories can spark real change - so, we’d love to hear yours.
